Annual Community Impact Reporting

Affiliates will enter data in two designated areas within REDCap annually. These forms capture essential program activities, outcomes, and school community impact across the national network.

REDCap Access Requirements & Account Setup

All affiliates must have an active REDCap user account to enter data. To obtain access, Request a REDCap account from the Project ADAM National Office. Each affiliate program is responsible for maintaining current access for staff who support annual data entry.

AED Report Form

Project ADAM Affiliate Programs work closely with partner schools, community sites, and youth sports organizations (when applicable) to collect information whenever a cardiac emergency response is activated. These data help inform national reporting on community impact, including appropriate responses to Sudden Cardiac Arrest and lives saved. Affiliates should follow the Data Entry Process outlined above to ensure accurate and timely entry into REDCap.

The AED Report Form is completed by the school or community site where the emergency occurred. Once the form is submitted to the local Project ADAM Affiliate Program, the affiliate is responsible for:

  • Reviewing the submitted information

  • Ensuring completeness and accuracy

  • Entering the data into the National/WI REDCap database annually

This form is also available on the Project ADAM National website for easy access by schools and community partners.

Community Impact Form

Enter Annual Community Impact data into the National/WI REDCap database. Here is a copy of the Community Impact Data, due December 31 each calendar year. This data captures key program metrics: Recognized Heart Safe Schools, Educational & Professional Presentations, CPR‑AED training and community outreach.
